The senior care directory industry sells badges. We don't. Every Placet Award is computed from public Medicare data with a published methodology, an exact SQL query, and an enforced exclusion list. Operators cannot pay, lobby, or apply.
Awards are recomputed annually from the most recent CMS Care Compare snapshot. Facilities flagged for abuse, designated as Special Focus Facilities, or with a substantiated complaint in the past 12 months are excluded from every category.
Skilled nursing facilities whose registered nurse coverage on weekends is at least 85% of their weekday RN coverage AND whose staffing rating is 4 stars or higher AND whose total nursing hours per resident day are at or above the state median. The facility does not slack on Saturday.
Top 10% of nursing facilities in each state by composite CMS quality score, filtered to facilities with at least one verified clinical capability (tracheostomy, ventilator, wound vac, IV antibiotics, dialysis, or bariatric care).
Facilities with zero deficiencies at scope/severity G or above for the past five inspection cycles. A "clean" survey under CMS does not mean perfect — it means no deficiency rose to actual harm or immediate jeopardy.
