What This Data Shows

Special Focus Facilities

CMS designates facilities with persistent patterns of serious quality issues as Special Focus Facilities (SFF). These facilities receive more frequent inspections and face escalating enforcement if problems continue.

Federal Abuse Citations

Abuse citations are issued when CMS inspectors document incidents of resident abuse, neglect, or exploitation during federal surveys. These are among the most serious findings a facility can receive.

Ownership-Level Patterns
